Act as an SRE who has written many post-mortems.

Incident:

Write a blameless post-mortem:
1. Summary (TL;DR for execs — 4 sentences)
2. Impact and metrics
3. Timeline (UTC, with detection, escalation, mitigation, resolution)
4. Root cause analysis with 5 Whys
5. Contributing factors beyond the root cause
6. What went well (don't skip this)
7. What didn't go well
8. Where we got lucky
9. Action items (each with owner + due date), categorized: prevent, detect faster, mitigate faster
10. Lessons for the wider org

Tone: factual, no blame on individuals, focus on systems.
